ABSTRAK Latar belakang: Hernia nukleus pulposus (HNP) adalah suatu keadaan dimana terjadinya penonjolan diskus intervertebralis yang menimbulkan penekanan pada medula spinalis yang dapat menimbulkan gejala nyeri dan mengganggu aktivitas. Faktor risiko terjadinya hernia nukleus pulposus adalah jenis kelamin, usia, pekerjaan, indeks massa tubuh, riwayat trauma,dan riwayat jatuh . Oleh karena itu, penelitian ini ditujukan untuk meneliti gambaran faktor risiko pada pasien hernia nukleus pulposus di RSUD INDRAMAYU Kasus HNP dengan indikasi Post operasi Lumbal IV di RSUD INDRAMAYU pada periode Agustus sampai dengan Desember 2021 ditemukan kasus ini sejumlah 6 kasus,walaupun angka nya tidak lebih dari 10 dibandingkan kasus lain kasus ini menarik bagi saya untuk mengambilnya. Tujuan pembuatan TA ini diharapkan mampu mendapatkan pengalaman yang nyata serta mampu mampu melaksanakan asuhan keperawatan secara langsung dan komperhensif,meliputi bio-psiko-sosial-spritual,pada kasus ini saya menggambil kasus HNP dengan pasien Ny.C diruang Cengkir 1 RSUD INDRAMAYU dengan pendekatan proses keperaawatan.Metode yang digunakan dalam penulisan ini yaitu metode deskriptif yang berbentuk laporan kasus dan pengumpulan data dengan cara: wawancara,observasi,pemeriksaan fisik,studi keperawatan dan dokumentasi.Sistematika penulisan ada 5 bab, yaitu : pendahuluan,teoritis,tinjauan pustaka,pembahasan dan kesimpulan. Asuhan keperawatan ini diambil pada tanggal 8-10 Desember 2021 di Ruang Cengkir1 RSUD INDRAMAYU. Kata kunci ; Asuhan keperawatan dengan kasus Hernia Nukleus Pulpousus (HNP) ABSTRACK Background: Hernia nucleus pulposus (HNP) is a condition where there is protrusion of the intervertebral disc which causes pressure on the spinal cord which can cause symptoms of pain and interfere with activities. Risk factors for hernia nucleus pulposus are gender, age, occupation, body mass index, history of trauma, and history of falls. Therefore, this study aimed to examine the description of risk factors in patients with hernia nucleus pulposus at INDRAMAYU Hospital. HNP cases with indications of Post Lumbar IV surgery at the INDRAMAYU Hospital in the period August to December 2021 found 6 cases, although the number was not more than 10 compared to other cases, this case was interesting for me to take. The purpose of making this TA is expected to be able to get real experience and be able to carry out direct and comprehensive nursing care, including bio-psychosocial-spiritual, in this case I took the HNP case with patient Mrs. C in Cengkir 1 RSUD INDRAMAYU with a process approach nursing. The method used in this writing is a descriptive method in the form of case reports and data collection by means of: interviews, observations, physical examinations, nursing studies and documentation. There are 5 chapters of systematic writing, namely: introduction, theoretical, literature review, discussion and conclusion. This nursing care was taken on December 8-10, 2021 in the Cengkir1 Room of the INDRAMAYU Hospital.
